Agency Application Instructions
Thank you for your interest in partnering with the Greater Cleveland Food Bank. Our mission is to work together to ensure that
everyone in our communities has the nutritious food they need every day.
Below are the steps to filling out an application. Please
make sure to include all attachments.
Member Agency Application
Copy of the 501(c)3 IRS determination letter
(This is not your tax exempt I.D. number, but a letter from the IRS confirming
your organization’s tax exempt status)
A list of your organization’s Board of
Directors, including their name, address and phone number
Letter of Interest (Must be on your
organization’s letterhead that includes a brief description of the program you
operate or plan on operating. Please include the “who, what, where, when, and
why” behind your organization’s mission and provide detailed information on how
the program will be funded)
Please send your completed application along with all
